Embolization of Cerebral Aneurysms with Stent and Detachable Platinum Coils-Single Center Experience

Abstract
Objective£ºThe authors reviewed the cerebral aneurysm embolization cases which used stent along with detachable platinum coils for their feasibility and effectiveness.

Methods£ºTotal 20 cases of cerebral aneurysm embolization cases were done using stent with detachable platinum coils from Dec. 2000 to Dec. 2005. We retrospectively analyzed the data including location, morphology, and rupture state of aneurysms, stent deployment, coil packing, follow-up study and complications using medical records and radiological images.

Results£ºMost of the aneurysms (17/20) emblized were found in unruptured state and located in the distal ICA (9/20) and top of the basilar artery (5/20). Their mean dome/neck ratio was 1.55. Coronary stents were used in the early 7 cases and Neuroform II (6/20), III (5/20), and Leo (2/20) stents replaced them. Most of the aneurysms were saccular in shape except 1 fusiform and 2 dissecting aneurysms. During the procedures, most of the stents were deployed optimally (covering more than 4mm of aneurysm neck both proximally and distally)(16/20) and before the packing of coils (15/20). Complete aneurysmal obliteration was attained only in 8 cases. Three thromboembolic events were occurred and one stent migration, vasospasm, postprocedure SAH was found in each.

Conclusion£ºStents can be used effectively and safely for cerebral aneurysm embolization especially in broad neck and compacted cases. But considering their complications another options such as balloon and multiple catheter technique should be considered deliberately before the decision of their usage.
